Morris County Health Needs Assessment

Community Health Assessment (or CHA) is a process for determining the needs in a particular community or population through systematic, comprehensive data collection and analysis, and leveraging results to spur community change. The CHA provides organizations comprehensive information about a community’s current health status, needs, and issues. In turn, providing information that can be used to develop a Community Health Improvement Plan (CHIP) by identifying how and where resources should be allocated to best meet community needs.

A Community Health Improvement Plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of a CHA and the community health improvement process. This plan is typically updated every three to five years. Get Involved!!! In collaboration, The Morris Regional Public Health Partnership and The College of New Jersey (TCNJ) are working on the completion of a county-wide CHA and CHIP. Participate by completing the following Community Needs Assessment Survey. Your input is important and will help guide the quality of health within your community!
